The South West CCAC provides support and resources for respite/caregiver support including:

  • Information and referral to community resources
  • System navigation
  • Access to long term care and day programs
  • Case management services including individualized assessment and Service Planning for in home services and supports

Two specific Respite/Caregiver Support programs that South West CCAC handles applications for are: Short Stay and Adult Day Programs.

The CCAC is an organization that is funded by the Ministry of Health and Long Term Care that manages local care.  The CCAC connects individuals to home care, long-term care destinations, and other services in your community.
