The Champlain Community Care Access Centre is the local point of access to community-based health care services. The Centre is funded by the Ministry of Health and Long-Term Care. It was created to coordinate a variety of health services to assist in maintaining an individual’s health, independence and quality of life. The CCAC connects individuals to home care, long-term care destinations, and other services in your community.
