The Home Support Program offers emotional support and practical help to patients facing a life-threatening illness who are being cared for at home. The Hospice’s professional staff supports trained volunteers who make weekly home visits. These volunteers provide visits and assist with shopping and meals.

Respite services provide relief and support to families who have children or adults with a developmental disability living at home.

Community Care Access Centre (CCAC)

The CCAC is an organization that is funded by the Ministry of Health and Long Term Care that manages local care.  The CCAC connects individuals to home care, long-term care destinations, and other services in your community.

The Supportive Housing Program, the largest and most cost effective in eastern Ontario, consists of six locations throughout Ottawa and provides a range of support services to adults with physical disabilities. Clients live independently in an unsupervised environment and based on individual card needs, services are provided in a residential setting or group home. Also manages the only community based respite for adults with physical disabilities.

Provides 24 hour on-call as well as scheduled attendant care services to adults with physical disabilities who are living in their own units within an apartment complex. The units provided by the housing provider are accessible and have been retrofitted to the needs of the consumer. Attendant service hours are customized to suit each individual lifestyle and to promote independence.


Ottawa-Carleton Association for Persons with Developmental Disabilities (OCAPDD) - Family Homeshare Program

The program has been designed to connect individuals with a developmental disability ("Homesharers") to families and individuals ("Homeshare Providers") in their home within the community. The Homesharer moves into the Homeshare Provider’s home for an indefinite period. Through this caring and shared experience, the Homesharer becomes a participant in the household and the community.
