The South West Community Care Access Centre is the local point of access to community-based health care services. The Centre is funded by the Ministry of Health and Long-Term Care. It was created to coordinate a variety of health services to assist in maintaining an individual’s health, independence and quality of life.

Community Care Access Centre (CCAC) help those who need care stay in his/her own home longer by providing Care in Your Home and by coordinating care in the community, including specialized support services
CCAC can also provide information about long-term care options once it has become too difficult to live independently at home.
