My Voice: Expressing My Wishes for Future Health Care Treatment

The Ministry of Health for the Province of British Columbia has recently developed a guide to advance care planning.  This tool has been developed for residents of British Columbia but would be helpful to residents of any province.  Go to their website at http://www.health.gov.bc.ca/hcc/advance-care-planning.html and click on the link in the page titled My Voice - Expressing My Wishes for Future Health Care Treatment.
